Care Plan: is a formal plan of care from a professional who performs a
comprehensive assessment in the elder’s home focused on: fall
prevention, medication safety, psycho-social and cognitive needs,
medical history/medical needs, activity levels and nutrition, human
support and financial resources available for care.

Care Management: are the steps necessary to bring the care plan to life
– care management can be executed by the professional who designed the
plan, another professional or the elder’s advocate.

General Advocacy: care supervision that requires the knowledge and
skills of a registered nurse or medical social worker to accomplish –
typically focused on long term care facility evaluation and facility
visits for quality assurance.

Medical Advocacy: care supervision that includes accompanying the elder
on medical appointments to improve care outcomes and facilitate
communication among providers such as assisted living and nursing homes.

surrogate family member to support client’s connections to community
activities and social network; facility and quality assurance visits.

Downsizing
A de-cluttering process to improve safety and assist an older adult and
his or her family in preserving memories, keeping cherished items and
heirlooms, and selling or gifting those items no longer needed.

Relocation and Move
Management
Provide a single point of contact to thoughtfully plan and coordinate all
aspects of a move from one home to the next. Services include referrals,
scheduling, estate sale coordination, and move-in assistance (furniture
placement, unpacking, organizing).
